Logo came out way too small and bad quality

Logo was too small making the embroidery look bad. The size was less than 2.5 inches instead of 3.5" like it usually is. We have never had this issue before. A Vistaprint design person helped with the design project as it wasn't adding to my cart properly due to a product update and so they created the design and added to my cart (the proof looked ok). I have sent in a request as per the chat agent's directions, and haven't heard back and it has almost been a week now. The jackets also came dirty even though they were in a bag.
